In other words, surgery is more likely to result in long-term remission if it is performed early after diagnosis.&nbsp;<\/span><\/span><span class=\"EOP SCXW178371210 BCX0\" data-ccp-props=\"{}\">&nbsp;<\/span><\/p>\n\n\n\n<p><strong>Evidence from clinical trials<\/strong><\/p>\n\n\n\n<p><span class=\"TextRun SCXW254978984 BCX0\" lang=\"EN-GB\" xml:lang=\"EN-GB\" data-contrast=\"auto\"><span class=\"NormalTextRun SCXW254978984 BCX0\">There have been at least two trials performed in patients <\/span><span class=\"NormalTextRun SCXW254978984 BCX0\">with <\/span><span class=\"NormalTextRun SCXW254978984 BCX0\">diabetes undergoing surgery that have clearly <\/span><span class=\"NormalTextRun SCXW254978984 BCX0\">demonstrated<\/span><span class=\"NormalTextRun SCXW254978984 BCX0\"> the short<\/span> <span class=\"NormalTextRun SCXW254978984 BCX0\">and <\/span><span class=\"NormalTextRun SCXW254978984 BCX0\">long-<\/span><span class=\"NormalTextRun SCXW254978984 BCX0\">term benefits of these procedures<\/span><span class=\"NormalTextRun SCXW254978984 BCX0\">,<\/span><span class=\"NormalTextRun SCXW254978984 BCX0\"> as compared to best medical treatment. The STAMPEDE trial has <\/span><span class=\"NormalTextRun SCXW254978984 BCX0\">demonstrated<\/span><span class=\"NormalTextRun SCXW254978984 BCX0\"> that up<\/span> <span class=\"NormalTextRun SCXW254978984 BCX0\">to <\/span><span class=\"NormalTextRun SCXW254978984 BCX0\">a third of patients have resolution of diabetes 3 years after surgery<\/span><span class=\"NormalTextRun SCXW254978984 BCX0\"> compared to best medical care. This was seen with<\/span><span class=\"NormalTextRun SCXW254978984 BCX0\"> both the gastri<\/span><span class=\"NormalTextRun SCXW254978984 BCX0\">c bypass and sleeve gastrectomy<\/span><span class=\"NormalTextRun SCXW254978984 BCX0\">.&nbsp;<\/span><\/span><span class=\"EOP SCXW254978984 BCX0\" data-ccp-props=\"{}\">&nbsp;<\/span><\/p>\n\n\n\n<p><span class=\"TextRun SCXW226863005 BCX0\" lang=\"EN-GB\" xml:lang=\"EN-GB\" data-contrast=\"auto\"><span class=\"NormalTextRun SCXW226863005 BCX0\">It is based on such evidence that the American Diabetes Association (ADA) has recommended surgery as a treatment modality of Type 2 diabetes. It is also the rationale behind the National Institute of Clinical Excellence (NICE) recent recommendations for bariatric surgery. She underwent the procedure and was started on a sliding scale<\/span><span class=\"NormalTextRun SCXW4019165 BCX0\"> (subcutaneous <\/span><span class=\"NormalTextRun SCXW4019165 BCX0\">administrations of insulin depending on the blood glucose level)<\/span><span class=\"NormalTextRun SCXW4019165 BCX0\"> in the immediate post-operative period.
