TdP is a specific type of electrical activity that particularly affects the ventricles (the larger chambers in the heart), causing them to contract too quickly. This condition is identified using an ECG (electrocardiogram; monitoring that shows the electrical activity in the heart).\u00a0 Torsades de Pointes literally means \u201c<em>twisting of the points<\/em>\u201d, which describes the classical pattern seen on the ECG. Women normally have a slightly longer QT interval than men. A longer QT interval can also be something you are born with or caused by other medical conditions or abnormalities:<\/p>\n<ol>\n<li>Congenital Long QT Syndrome\n<ul>\n<li>This is a relatively rare condition that around 1 in 2,000 people are born with<a href=\"#_edn1\" name=\"_ednref1\">[i]<\/a>, meaning that their QT interval is longer than normal. In people with LQTS, stress, strenuous exercise or startling can cause them to develop TdP<a href=\"#_edn2\" name=\"_ednref2\">[ii]<\/a>.<\/li>\n<\/ul>\n<\/li>\n<\/ol>\n<ol start=\"2\">\n<li>Cardiac problems\n<ul>\n<li>Other heart problems that are associated with prolonging the QT interval and increasing the risk of developing TdP include:\n<ul>\n<li>Valve disease<a href=\"#_edn3\" name=\"_ednref3\">[iii]<\/a><\/li>\n<li>Ischaemia<a href=\"#_edn4\" name=\"_ednref4\">[iv]<\/a> (having heart attacks or \u2018angina\u2019 type chest pain)<\/li>\n<li>Other problems with the electrical activity in the heart<a href=\"#_edn5\" name=\"_ednref5\">[v]<\/a><sup>,<a href=\"#_edn6\" name=\"_ednref6\">[vi]<\/a><\/sup> (known as \u2018arrhythmias\u2019), such as \u2018AVN block\u2019 and \u2018bradycardia\u2019 (having a slow heart beat)<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<\/li>\n<\/ol>\n<ol start=\"3\">\n<li>Medications\n<ul>\n<li>There are several medications that can prolong the QT interval, including<a href=\"#_edn7\" name=\"_ednref7\">[vii]<\/a>:\n<ul>\n<li>Drugs to treat arrhythmias (electrical problems in the heart)<\/li>\n<li>Several antibiotics and antifungals<\/li>\n<li>Antidepressants like citalopram or amitriptyline<\/li>\n<li>Some medications that are used to treat other psychiatric disorders<\/li>\n<li>Some anti-sickness medications like ondansetron<\/li>\n<\/ul>\n<\/li>\n<li>If you have any concerns regarding any of the medication you are taking, you should discuss this with your GP<\/li>\n<\/ul>\n<\/li>\n<\/ol>\n<ol start=\"4\">\n<li>Electrolyte imbalance\n<ul>\n<li>Very low levels of the following electrolytes can cause TdP:\n<ul>\n<li>Potassium<\/li>\n<li>Calcium<\/li>\n<li>Magnesium<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<\/li>\n<\/ol>\n<p>These are often caused by severe or prolonged vomiting and\/or diarrhoea or severe malnutrition, but can also be caused by some types of endocrine disorders.<\/p>\n<ol start=\"5\">\n<li>Other\n<ul>\n<li>Rarely, TdP can be caused by\n<ul>\n<li>Thyroid disease<a href=\"#_edn8\" name=\"_ednref8\">[viii]<\/a><sup>,<a href=\"#_edn9\" name=\"_ednref9\">[ix]<\/a><\/sup> (either hyperthyroidism or severe hypothyroidism)<\/li>\n<li>Exposure to toxins such as insecticides<a href=\"#_edn10\" name=\"_ednref10\">[x]<\/a> or arsenic<a href=\"#_edn11\" name=\"_ednref11\">[xi]<\/a><\/li>\n<li>Hypothermia<a href=\"#_edn12\" name=\"_ednref12\">[xii]<\/a><em><u>\u00a0<\/u><\/em><\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<\/li>\n<\/ol>\n<p><strong>How do you treat TdP?<\/strong><\/p>\n<p><strong>Treatment of TdP<\/strong> depends on whether someone is currently having an episode of TdP or generally at risk of developing TdP (i.e. they have risk factors that mean they have a long QT interval).<\/p>\n<p>Immediate treatment:<\/p>\n<ul>\n<li>People with TdP are treated following national resuscitation guidelines<\/li>\n<li>If they are in hospital this involves:\n<ul>\n<li>Removal of anything that could have triggered the TdP (such as QT interval prolonging drugs)<\/li>\n<li>Defibrillation<\/li>\n<li>Administration of magnesium sulphate<\/li>\n<li>Correction of any abnormal heartbeat or electrolyte imbalance, sometimes using drugs or \u2018pacing\u2019 (taking over the electrical activity of the heart)<\/li>\n<\/ul>\n<\/li>\n<li>If they are not in hospital, the normal basic life support guidelines should be followed:<a href=\"#_edn13\" name=\"_ednref13\">[xiii]<\/a>\n<ul>\n<li>Ensure it is safe to approach the victim<\/li>\n<li>Promptly assess the unresponsive victim to determine if they are breathing normally<\/li>\n<li>Be suspicious of cardiac arrest in any patient presenting with seizures and carefully assess whether the victim is breathing normally<\/li>\n<li>For the victim who is unresponsive and not breathing normally:\n<ul>\n<li>Dial 999 and ask for an ambulance. If possible stay with the victim and get someone else to make the emergency call<\/li>\n<li>Start CPR and send for an AED (automated external defibrillator) as soon as possible (there should be one in most public places with clear, simple instructions for use)<\/li>\n<li>If trained and able, combine chest compressions and rescue breaths, otherwise provide compression-only CPR<\/li>\n<li>If an AED arrives, switch it on and follow the instructions<\/li>\n<li>Minimise interruptions to CPR when attaching the AED pads to the victim<\/li>\n<li>Do not stop CPR unless you are certain the victim has recovered and is breathing normally or a health professional tells you to stop<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<p>Long-term treatment:<\/p>\n<ul>\n<li>For people <em>with<\/em> congenital LQTS, the following is recommended<a href=\"#_edn14\" name=\"_ednref14\">[xiv]<\/a>\n<ul>\n<li>Ensuring a good diet with enough hydration and a variety of fruits and vegetables (to avoid any electrolyte imbalances)<\/li>\n<li>Avoidance of any drugs that could cause a longer QT interval<\/li>\n<li>Beta-blockers (such as propranolol)<\/li>\n<li>Permanent pacing with an ICD pacemaker (Implantable Cardioverter Defibrillator)<\/li>\n<li>If these treatment options are not working well enough, or the person is at a high risk of developing TdP, they can have an operation called a \u2018Sympathectomy\u2019 to help control the heart\u2019s rhythm<a href=\"#_edn15\" name=\"_ednref15\">[xv]<\/a><\/li>\n<\/ul>\n<\/li>\n<li>For people <em>without<\/em> congenital LQTS, the following is recommended:\n<ul>\n<li>Ensuring a good diet with enough hydration and a variety of fruits and vegetables (to avoid any electrolyte imbalances)<\/li>\n<li>Correction of any reversible risk factors that could cause long QT interval<\/li>\n<li>In cases where these factors cannot be avoided, or in the case of other heart rhythm problems, pacemakers can be implanted to prevent TdP<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<p><a href=\"#_ednref1\" name=\"_edn1\"><\/a><\/p>\n<p><a class=\"popmake-think-meningitis-not-just-a-rash\" href=\"#\">References<\/a><\/p>\n<h6 class=\"p1\"><span style=\"color: #808080;\"><span class=\"s1\">This article is for information only and should not be used for the diagnosis or treatment of medical conditions. myHealthSpecialist makes no representations as t<\/span><span class=\"s2\">o the accuracy or completeness of any of the information in this article, or found by following any\u00a0link from this article<\/span><span class=\"s1\">.
