Mr Machesney is the chair of the Colorectal Cancer Clinical Reference Group (CRG) for NHS England, which aims to improve patient experience, safety, and early diagnosis. He is also the Pathway Director for Colorectal Cancer at London Cancer, an integrated cancer system resulting from the amalgamation of the North East London Cancer Network (NELCN) and the North Central London and West Essex Cancer Network (NCLWECN) following the Cancer Review for London (CRL). He is involved with National Peer Review, has been a CQC Hospital Inspector, and is on the Clinical Advisory Board of Bowel Cancer UK.

He has been in consultant practice as a surgeon since 2003. For the first nine years of his appointment, he was the Lead Clinician for Colorectal Cancer at Whipps Cross University Hospital NHS Trust, where he built a team that was an early adopter of laparoscopic surgery and enhanced recovery for colorectal patients. Whipps Cross was recorded in the CRL Case for Change document (2008) to have had the second highest rate for laparoscopic surgery and the lowest length of stay for colectomy in the capital. He also commissioned and participated in the implementation of a triage straight-to-test pathway for patients with colorectal symptoms at Whipps Cross, which was shortlisted for a BMJ Award in 2014.

Mr Machesney's areas of expertise include colonoscopy, endoscopy (upper GI endoscopy/gastroscopy), colorectal surgery (both open and laparoscopic), banding of haemorrhoids, laying open of anal fistula, hernia repair (open and laparoscopic), excision of skin lesions, and gall bladder removal (open and laparoscopic cholecystectomy). He treats conditions such as bowel cancer, gallstones, hernia, haemorrhoids, anal fissure, and fistula.
